Terms and Conditions of sign-up to My Healthy Warwickshire

Collection and use of personal information.

We shall collect and use any personal information you give to us in accordance with the site’s privacy policy, terms and conditions and the statements below. We shall not disclose your personal information to any third parties without obtaining your prior consent unless we are required by law to do so.

In addition you may submit information about your diet and lifestyle, including drinking or exercise habits. By submitting this information, you consent to NHS Warwickshire using it for the purpose of offering relevant content to you on the website and, if you so chose, by email.
  2. Additionally, you may submit health information, including your height and weight, or declare your interest in one or more health goals, conditions or treatments. By submitting this information you explicitly consent to NHS Warwickshire using the information, or any information we may infer from it, to provide relevant content and information to you in your account pages or, if you so chose, by email.
  3. From time to time, NHS Warwickshire may wish to use the information you have submitted for research purposes or producing statistics. With your permission, academic institutions may occasionally use our data in relevant studies. In these instances, we shall inform you in advance and you will have the choice to opt out of the study. The information that is used will be made anonymous and will be confidential.

NHS Warwickshire forums

The NHS Warwickshire forums are aimed at supporting people in living more healthy lifestyles and in getting involved with the NHS in this region. They are intended to provide a warm and supportive environment for the online Warwickshire community to express their views and concerns.

Rules of Conduct

  1. Be respectful of others at all times.
    NHS Warwickshire forums are designed to provide a supportive platform for users to communicate with NHS Warwickshire and to share their health experiences. Whilst debate and discussion is fine, we will not sanction rudeness, insulting posts or personal attacks. There will be occasions where the views and perspectives of individual members are different; please be courteous when disagreeing with others. Please also remember that the meaning of phrases when written in an electronic format can often be misconstrued. Jokes can often end up causing offence.
  2. Personal attacks (Flaming) are banned.
    Personal attacks are messages which are designed to insult or offend the reader of a message. This doesn’t have to be directed at a specific individual. Posts of this type are not beneficial to open debate and will not be tolerated.
  3. "Trolling” i.e. the posting of messages designed to deliberately anger, insult or offend, will not be tolerated.
    “Trolling” will result in NHS Warwickshire restricting access to online forums.
  4. Do not post personal information.
    Please respect the privacy of other forum users and do not submit your own personal details to the forum. The moderator will monitor all messages to ensure that they are not of a personal nature.
  5. Non-commerciality.
    We do not allow forum users to utilize the forum as a platform for advertising commercial websites, products or services. Forums are for informational purposes only.
  6. Restrictions: warnings or bans.
    Any warnings issued by the forum moderators remain private between the moderator and the user. Questions or comments about individual cases should be submitted via email and not become the subject of a thread.
  7. Instant restriction.
    Forum users can/will be instantly “banned” where the following activities have been engaged in:
    • The posting of pornography
    • The posting of discriminatory remarks, sexually-explicit comments, harmful,
    • threatening or abusive comments
    • The posting of comments which are obscene, defamatory, hateful, racially or ethnically offensive
    • The posting of aggressive, violent or vulgar language
    • The posting of spam
    • Any post which discusses illegal activity
    • Any post which provides a link to sites which contain any of the above
  8. Constructive Feedback.
    Tell us what you think about the forums. Even negative comments can be very useful. Please ensure that these are presented in a civil and factual manner.
  9. Username.
    Users may register for one My Healthy Warwickshire account. Multiple aliases are not permitted. We also do not allow users to register with multiple email accounts. If a member is found to be using more than one alias, all their accounts will be terminated immediately. We reserve the right not to allow a username that might be deemed offensive. In addition we will not allow forum users to register with a commercial username. We will also not allow users to register usernames that are intended to point other forum users towards specific websites, be they commercial or personal.
  10. Images.
    The selection of an avatar image is permissible from the stock collection. We do not allow users to upload images from their own computers / collection.
